Where required, Swift Medical will take the responsibility for customs processing of our shipments.
If you are contacted regarding any of the above items, please reply that Swift Medical should be the Importer of Record, all necessary customs registrations have already been completed by Swift Medical, and the customs brokerage should be assigned to Carson International (pars@carson.ca)
If this does not resolve the issue, please forward the email you received directly to orderhealx@swiftmedical.com and we will follow up.
All products labelled as "HealX" are the same HealX sticker products, and the only difference is how they are packaged. Based on customer feedback, Swift provides the HealX product in different packaging formats for ease of use.

We are a Canadian company and therefore we use W8-BEN rather than a W9. To get this form, please email a request to orderhealx@swiftmedical.com and we will gladly send one over.
Swift Medical will always make our best effort to notify our customers of any known issues with the store website.

Please send documentation that shows your tax exempt status (such as a form or certificate) to orderhealx@swiftmedical.com, and we can apply a tax exempt status to your account.
